CVS’s Zepbound U-Turn Isn’t Just a Lilly Win – It’s the GLP-1 Market’s Tipping Point

The GLP-1 market has been stuck in a duopoly access war for over a year. Patients faced narrow coverage choices, and drugmakers fought for PBM formulary slots. Lilly took the biggest hit last year when CVS picked Novo’s Wegovy over Zepbound. The decision shaved nearly 12% off LLY stock in a single trading session. Almost no one expected CVS to reverse that call this fast.

On May 28, CVS Caremark announced it will cover Zepbound starting October 1. Its new oral GLP-1 Foundayo gets coverage starting June 1, just two months after FDA approval. All three top U.S. PBMs now cover Lilly’s full obesity drug portfolio. CVS says eligible patients will get equal access to Novo and Lilly products at the same co-pay. Lilly’s Q1 2026 revenue jumped 56% YoY to $19.8 billion, adjusted EPS up 156% to $8.55. LLY stock rose 4% on the news, now sits less than 5% below its all-time high.

The coverage expansion removes the biggest barrier to Foundayo’s rollout. 80% of Foundayo’s current users have never taken GLP-1 drugs before, opening an untapped patient pool. Cheaper compounded Zepbound copies from telehealth firms will lose their price edge as insurance covers the branded version. Lilly’s forward P/E sits at 29x, well below its 3-year average of 43x, leaving clear room for upside. The GLP-1 market’s duopoly balance will shift sharply in Lilly’s favor over the next 12 months.
